1. Medical Practice
Acumen Health Institute PLLC is a physician-owned medical practice providing telemedicine services to eligible adult patients.
Our services include physician consultations, comprehensive metabolic health assessments, ongoing medical management, nutrition and lifestyle counseling, weight management, and evidence-based treatment of metabolic conditions.
2. Eligibility
You must:
Be at least 18 years of age.
Provide accurate and complete information.
Be physically located in a state where the treating physician is licensed at the time of your telemedicine visit.
We reserve the right to decline or discontinue services when medically, legally, or ethically appropriate.
3. No Emergency Services
Acumen Health Institute PLLC does not provide emergency medical care.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
Do not rely on electronic messaging or scheduled appointments for urgent medical concerns.
4. Physician–Patient Relationship
Visiting this website does not establish a physician–patient relationship.
A physician–patient relationship is established only after:
Registration as a patient,
Completion of required intake documentation,
Review and acceptance by Acumen Health Institute PLLC,
Completion of an initial physician evaluation.
5. Telemedicine Services
Medical services are provided through secure HIPAA-compliant telemedicine technology.
You acknowledge that telemedicine has limitations, including potential technical issues, and may not be appropriate for every medical condition.
If your physician determines that in-person evaluation or emergency care is necessary, appropriate recommendations will be made.
6. Medical Decisions
Treatment recommendations are individualized.
Not every patient is an appropriate candidate for every treatment, medication, dietary approach, or diagnostic test.
All medical decisions remain solely at the discretion of the treating physician based on clinical judgment.
7. Prescriptions
Prescription medications are issued only when medically appropriate following physician evaluation.
Submission of payment, completion of questionnaires, or scheduling an appointment does not guarantee that any medication—including GLP-1 medications—will be prescribed.
Acumen Health Institute PLLC does not sell or dispense medications directly.
8. Fees and Payment
Fees for services are disclosed before purchase.
Payment is generally due at the time appointments are scheduled unless otherwise specified.
Recurring membership fees are billed according to the selected membership plan.
Failure to maintain payment may result in suspension or termination of membership services.
9. Membership Services
Membership benefits apply only while an active membership is maintained.
Membership terms, included services, and pricing may change prospectively with reasonable notice.
Patients may cancel membership according to the Cancellation & Refund Policy.
10. Cancellations and Refunds
Appointment cancellations, missed appointments, membership cancellations, and refund requests are governed by our separate Cancellation & Refund Policy, which is incorporated into these Terms by reference.

Information provided on this website is intended for educational and informational purposes only.
Website content should not be interpreted as individualized medical advice.
Medical recommendations require evaluation by a licensed physician.
